I don't believe the section "Initial setup" is valid or relevant.

Certainly with Wheezy the keys are regenerated at first run, though I don't know about the other distributions.

To confirm see the file:

/var/log/regen_ssh_keys.log

On my system it shows:

Creating SSH2 RSA key; this may take some time ...
Creating SSH2 DSA key; this may take some time ...
Creating SSH2 ECDSA key; this may take some time ...
insserv: warning: current start runlevel(s) (empty) of script `ssh' overwrites defaults (2 3 4 5).
insserv: warning: current stop runlevel(s) (2 3 4 5) of script `ssh' overwrites defaults (empty).
update-rc.d: using dependency based boot sequencing
update-rc.d: using dependency based boot sequencing
finished
Starting OpenBSD Secure Shell server: sshd.

The file is has the date of first run of the operating system. A quick visual check of the key on two different systems also shows the files to be different.

I propose that section should be removed from the document.

If this may be the case for only certain distributions then perhaps update the section to state that - or raise this as an bug against those distributions.
